This document officially records the behavioural problem(s) of the above named student. After this warning the student will be on a probationary status and their parents will be informed of their behaviour.

Reasons for probation : 15 june 2008

EF staff member found the student from paceville at villa rosa beach after the curfew at 00:00. As the curfew on sunday is at 00:00. Student lives with a host family and a taxi ride to the host family takes from 10 to 15 minutes.

Note: curfew on monday 16 june 2008 is at 8:00 pm .
